Welcome to our office, Consultants in
Gastroenterology, P.C.
We appreciate the opportunity to provide you with
medical services.
The specialty of Gastroenterology encompasses
diseases of the digestive system, gallbladder, liver, and pancreas
as well as clinical aspects of nutrition. Our physicians
are Board Certified in Internal Medicine and Gastroenterology.
Gastroenterologists are not surgeons, but they do
perform diagnostic studies such as colonoscopy, flexible
sigmoidoscopy, and gastroscopy; along with more specialized
procedures.
Office Location
Our office is conveniently located in the office complex
next to the Lincoln Surgery Center. Our receptionists answer the
phones from 8:30 AM until 4:30 PM, Monday through Friday. Patient
parking is conveniently located next to the Lincoln Surgery Center.
Appointments
Appointments may be made by calling our office at
441-5600.
If you are referred to our office by your primary care
physician, he/she will receive a formal report including diagnosis,
test results and recommendations for treatment and follow-up.
Additional follow-up visits at our office will be scheduled as
needed.
It is not necessary to be referred by another
physician to be seen in our office. However, your problems
must be gastrointestinal in nature and we encourage you to have a
family physician or internist to manage your routine and general
medical care.
If you are unable to keep your scheduled appointment,
please notify the office at least 48 hours in advance to
reschedule. This allows other patients to use your time
slot.
Telephone Calls
If you need to speak with your physician, the nurse
practitioner, or the nurse please give your name, phone number and a
brief description of what you need. Someone will return your
call as soon as possible. Please understand that the
physicians and nurses are often with patients when you call,
therefore calls may not be returned immediately.
Prescription Refills
If you need a refill of a medication you are currently
taking, please contact your pharmacist several days before you run
out of your medication. If an authorization to refill your
medication is needed, your pharmacist will contact our office and
get the required information.
Emergency Calls
We recognize that you can have an emergency situation
arise and we will do our best to respond to your problem
promptly. In the event of a severe situation, go immediately
to the emergency department of the hospital or call 911. If
the situation is not that severe, but one where you wish to speak to
the physician, please call our office at 441-5600 during regular
office hours, or call our answering service at 473-3813 after office
hours. The answering service will forward messages to the
physician on call.
Medical Confidentiality
All of our records are kept in strict confidence and
will be released only with your written permission. (print a
release
form)
Fees and Payments
The fee for your
initial visit will vary depending on the complexity of your
problem. There is a fee
for your examination, laboratory tests, and any additional
procedures that may be required during the course of your
care.
You
are expected to pay any co-pay, deductible, or co-insurance at the
time of service.
For your convenience, we accept cash, check, Visa, and
Mastercard.
You may arrange a payment plan as necessary for large account
balances.
Insurance and Medicare
As a new patient,
please bring your insurance cards with you at the time of your
appointment so we can make a copy for our records. If your insurance coverage
changes, you must notify our office and allow us to copy your new
insurance cards.
We participate
with the following insurance companies:
As a
courtesy, we will directly file commercial insurance forms when you
provide us with all necessary insurance information.
After
receiving notification from your insurance company, we will bill you
for the remaining balance due.
Balances are due 30 days following the receipt of the
statement. If you are
unable to make payment in full, contact our office to set up payment
arrangements.
Each
insurance company has its own policies regarding payment. You should check with your
insurance company to determine whether you need authorization for
treatment, pre-certification, or a second opinion. It is absolutely necessary
that you inform our office staff of this. Failure to do so may result
in non-payment from your insurance company. You may then be held
responsible for any balance incurred.
